RHTP Hub Program Coordinator
TCHD is accepting applications for a full-time, non-exempt RHTP (Rural Health Transformation Program) Hub Program Coordinator to support Missouri’s Transformation of Rural Community Health Care (ToRCH Care) initiative.

TCHD is accepting applications for a full-time, non-exempt RHTP (Rural Health Transformation Program) Hub Program Coordinator to support Missouri’s Transformation of Rural Community Health Care (ToRCH Care) initiative. The RHTP Hub Program Coordinator will serve as the operational lead for the local rural health Hub and will be responsible for organizing, implementing, and managing day-to-day Hub operations.
This position will bring together healthcare providers, public health agencies, behavioral health organizations, emergency medical services, pharmacies, schools, and community-based organizations to improve access to coordinated clinical, behavioral, and social services for rural residents. The RHTP Hub Program Coordinator will facilitate Leadership Board activities, coordinate local program implementation, monitor Hub progress and performance, oversee RHTP Navigator workflows, support referral pathways, and ensure care coordination processes function effectively across participating organizations.
The ideal candidate is collaborative, organized, self-motivated, and passionate about improving healthcare access and outcomes in rural communities. The successful candidate will possess strong communication, facilitation, relationship-building, and organizational skills and will be comfortable coordinating multiple projects, working with diverse community partners, using data to support decision-making, and representing the Hub at meetings and community events.
Applicants must possess an associate degree or equivalent combination of education and experience, along with one to three years of experience in healthcare operations, public health, care coordination, social services, or a related field. Preference will be given to candidates with a bachelor’s degree in public health, healthcare administration, social work, health informatics, or a related discipline, as well as experience in rural healthcare, Medicaid programs, community-based initiatives, and value-based care.
TCHD provides a comprehensive benefits package which includes paid vacation (80 hours), holidays (9 Holiday and 3 Floating Holidays), and sick time (96 hours); LAGERS Retirement system (currently contributing 19.5 %) and 100% Paid Health Insurance for the employee.
